Medical – for own serious health condition
Under the FMLA, eligible team members are provided with up to 12 weeks of unpaid, job-protected leave per rolling calendar year when you are unable to work due to your own serious health condition. The link below will be directed to the Machine Readable Files that are required to be made available pursuant to the federal Transparency in Coverage Rule. Once re-directed, the files shall provide negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The Machine Readable Files are formatted to allow researchers, regulators, and application developers to more easily access and analyze data.
