
Life/AD&D and Disability Benefits
Cooper University Health Care has selected New York Life as our Life and Disability Carrier effective January 1, 2023. This change will allow you to buy Supplemental Life coverage up to Guarantee Issue amounts without having to provide Evidence of Insurability (EOI) up to below amounts:
This is a great opportunity to protect your loved ones with financial security if you’re no longer there to support them. If you are currently enrolled in voluntary disability and supplemental life plans, your current elections will automatically be continued with New York Life.
Supplemental Life Insurance
You have the option of purchasing supplemental life insurance for:

Yourself - up to five times your annual base pay in increments of $10,000 up to a maximum of $1,000,000.

Your Spouse - $5,000 to $25,000, in $5,000 increments.

Your Child(ren) - In the amount of $5,000 or $10,000.
Voluntary Short-Term Disability Benefit
Team Members who are eligible for voluntary STD as a supplement to the New Jersey Temporary Disability Benefits, can receive up to 66% of your weekly salary, in conjunction with your State Disability Benefits. You can choose among three (3) different waiting periods (30, 60 and 90-days), and will pay less premium for longer waiting period. You should consider how much Extended Sick Leave (ESL) you have when electing benefit waiting period. To view your ESL and PTO balances visit the ESS website or check your most recent pay-stub.
